What To Bring

For Covenant Clinic patients or Non-Covenant Clinic patients here’s what you need
to bring with you to the Flu Shot Clinic at the Kimball Ridge Center and Cedar Falls Senior Center:

• $25 cash or check
  
• Medicare Card
  
• WFHC employee dependents should bring their insurance card and at check in identify yourself as a dependent. The Wheaton Insurance plan will cover the $25 cost.
  
• WFHC volunteers over the age of 65, please bring with you your Medicare card.
